Based on your evaluation, your provider determines the appropriate membership tier and extends a personalized invitation to enroll. If you become a member, the $250 evaluation fee is applied toward your first month's membership.

Primary Care Physician

Dr. Rachel Okabe, MD

Dr. Okabe is a board-certified internal medicine physician and the clinical lead at HI Direct Primary Care. She graduated from NYU Grossman School of Medicine and completed her residency at NYU Langone Hospital in Brooklyn, where she also practiced primary care before returning to Hawaiʻi.

Dr. Okabe is the co-author of 19 peer-reviewed publications in journals including Cancer Cell and Nature Medicine, and holds additional training as an Integrative Nutrition Health Coach. Patients consistently describe her as someone who takes the time to actually listen.

Medical Director

Dr. Ira Zunin, MD, MPH

Dr. Zunin serves as Medical Director of HI Direct Primary Care. Over 35 years, he co-founded and led Manakai O Malama, caring for hundreds of thousands of families across the islands and building one of Hawaii's most trusted networks of specialist relationships.

Dr. Zunin now focuses on clinical oversight and research, including serving as principal investigator for active Phase 3 clinical trials through the Body and Mind Research Institute.

Body and Mind Research Institute

HI Direct Primary Care is affiliated with the Body and Mind Research Institute, LLC — an active hub for Phase 3 clinical trials providing data to the FDA for new drug approvals. As a member, you may have opportunities to participate in cutting-edge medicine, including current trials for next-generation weight management and diabetes medications. Participation is always voluntary.

What is Direct Primary Care?
Direct Primary Care (DPC) is a membership model where you pay a flat monthly fee for access to a primary care physician — no per-visit billing, no insurance middleman for primary care services. Your membership covers routine visits, preventive care, telehealth, and direct communication with your provider.
Is this a replacement for health insurance?
No. DPC is not a substitute for health insurance. You'll still need a traditional plan to cover hospitalizations, emergency care, surgeries, specialist visits, imaging, and other lab work. HI Direct Primary Care covers your primary care relationship — the ongoing, day-to-day connection with a provider who knows you.
How many in-person visits are included?
Your membership includes up to 6 in-person visits per year — each approximately 45 to 50 minutes long. Telehealth appointments and direct communication are included. If you need additional in-person visits beyond your 6, those are available for an added fee.
What happens at the initial evaluation?
The $250 initial evaluation is a one-on-one visit with your provider to discuss your health history, goals, and what you're looking for in a primary care relationship. Your provider uses this time to get to know you and determine whether HI Direct Primary Care is the right fit — and if it is, the $250 applies toward your first month's membership.
How is my monthly membership amount determined?
Membership starts at $200/month. After your initial evaluation, your provider determines the appropriate level based on your health complexity and anticipated care needs. You'll receive a personalized membership invitation with your tier once that determination is made.
How quickly can I get an appointment as a member?
Members can typically schedule in-person appointments within 2 to 3 business days. For non-urgent questions, telehealth, or messaging, you can reach your care team directly and expect a same-day or next-day response.
Can I communicate with my provider between visits?
Yes. Direct communication with your care team — by text or email — is included in your membership. We're not a 24/7 on-call service, but you'll have straightforward access to your provider for non-emergency questions and follow-up between visits. Expect a response within 24-48 hours from your provider.
What if I need more than 6 in-person visits?
Additional in-person visits beyond your annual 6 are available for a per-visit fee. Your care team will let you know as you approach your visit limit and can discuss whether a telehealth visit might serve your needs instead.
Do you see patients on the neighbor islands?
Yes. We typically recommend 1 to 2 initial in-office visits on Oahu to establish your care relationship, with telehealth for ongoing follow-up. If you're on a neighbor island, mention that when you book and we'll discuss the options that work best for you.
Are you accepting former Manakai O Malama patients?
Yes — verified former Manakai O Malama patients are given priority. There is also a benefit for verified former patients when you enroll. Please mention your Manakai history when you book your evaluation.
Does my insurance cover DPC membership fees?
HI Direct Primary Care does not bill insurance for primary care services, and monthly membership fees are not typically covered by insurance. Your insurance can still be used for prescriptions, diagnostic testing such as blood work and imaging, and referrals to specialists outside of this practice.
